Steps to Earn a PTE Office System Survey (this program is available for 2011):

1.  Review PTE OSS program and must-pass requirements (e.g. Care Management) using "PTE OSS Practice Worksheet and Scoring."

2. Complete the online OSS survey at: http://surveymonkey.com/s/WBSV9LV and the Maine Provider Database survey at: http://www.surveymonkey.com/s/MaineProviderDatabase_DataInput

3.  Review, sign "2010 PTE OSS Agreement and Cost," and mail payment to MHMC.  The cost is per physician per practice site.

4.  Once the above 3 steps are completed, Stephanie Pratt and the OSS assessor will connect with practices to coordinate the on-site assessment.
