Advanced Primary Care

The Maine Health Management Coalition’s Pathways to Excellence initiative has a new “Advanced Primary Care” recognition program designed to recognize those practices moving towards becoming patient-centered medical homes.  Two components are currently being reported on the MHMC website, www.getbettermaine.org, and 5 more are being offered in 2011-13.  The initial public reporting of these new components will be for participation, with eventual public reporting of results of care.  The components that will be reported are:

Safe / Office Systems Clinical Outcomes (one must be Depression) Patient Experience Including Access to Care (CG-CAHPS) Cost / Efficiency
e.g., CMS Meaningful Use, NCQA Patient-Centered Medical Home, BTE Medical Home e.g., NCQA Heart/Stroke or Diabetes recognition, BTE COPD, Hypertension, or other clinical recognition. Better or Best Total Cost of Care / Resource Use
