Advanced Primary Care
The Maine Health Management Coalition’s Pathways to Excellence initiative has a new “Advanced Primary Care” recognition program designed to recognize those practices moving towards becoming patient-centered medical homes. Two components are currently being reported on the MHMC website, www.getbettermaine.org, and 5 more are being offered in 2011-13. The initial public reporting of these new components will be for participation, with eventual public reporting of results of care. The components that will be reported are:
| Safe / Office Systems | Clinical Outcomes (one must be Depression) | Patient Experience Including Access to Care (CG-CAHPS) | Cost / Efficiency |
| e.g., CMS Meaningful Use, NCQA Patient-Centered Medical Home, BTE Medical Home | e.g., NCQA Heart/Stroke or Diabetes recognition, BTE COPD, Hypertension, or other clinical recognition. | Better or Best | Total Cost of Care / Resource Use |
